Step 1 - Call our Office

Call our office to complete an intake

The individual seeking treatment will need to call our office at, (731) 423-3653 during regular business hours and request an intake. At this time, you will provide your name and contact number for a member of our Admissions Dept. to call you to complete your intake screening. Due to privacy, we are only able to speak with the individual who is interested in seeking treatment.

Step 2 - Complete intake with Admissions

Complete the intake with our admissions staff

An intake is a brief set of questions that will help determine eligibility. A member of our admissions team will call you back on the same day, between 1 p.m. - 3 p.m. If we have received a high volume of calls, you may receive a call on the following business day to complete your intake.
